This retrospective cohort study examined the quality of written discharge communication for patients with dementia who are discharged from the hospital to a Skilled Nursing Facility. The study documents high rates of omission of information regarding neuropsychiatric symptoms and related care management strategies, which may hinder safe and effective transitions for this vulnerable population.
